By submitting this form, I authorize PromoteSafety Inc. to act on my behalf for limited administrative purposes related to the establishment of my business.

1. Business Formation & EIN Filing Authorization

I authorize PromoteSafety Inc. to:

  • Prepare and submit applications and documents necessary to form my business entity (including LLC, Corporation, or other entity type as instructed);
  • Prepare and submit IRS Form SS-4 and any related documentation necessary to obtain an Employer Identification Number (EIN);
  • Communicate with state agencies, the Internal Revenue Service (IRS), and other relevant authorities as necessary to complete these filings.

This authorization is strictly limited to administrative filings related to business formation and EIN registration. PromoteSafety Inc. does not provide legal or tax advice.

I understand that I am responsible for providing accurate information and for maintaining ongoing compliance obligations required by law.

2. Payment Authorization & Card-on-File Consent

I authorize PromoteSafety Inc. to charge my payment method for the PromoteSafety processing fee at the time of submitting this form.

I further authorize PromoteSafety Inc. to securely collect and store my payment method and to charge the same payment method at a later time for required government filing fees once filing amounts are finalized (not applicable for Sole Proprietorships filings only).

I understand and agree that:

  • The PromoteSafety processing fee is charged at submission of this form;
  • Required government filing fees are charged later to the payment method provided (if applicable);
  • Service fees, government filing fees, and payment processing fees are non-refundable once charged, regardless of filing outcome;
  • Government filing fees are set by the applicable state or agency and are non-refundable once submitted;
  • Payment processing fees charged by third-party processors are non-refundable;
  • Some states require payment directly through their own portals and may charge separate convenience fees beyond the control of PromoteSafety Inc.
